Credited where credit NOT due

I have been credited for working on productions I didn't actually work on. On several occasions I have requested these credits be removed from my page. I don't have request numbers for these, however, here are the credits that need to be removed:

Tarzan 2: Who Am I? (Video) (additional assistant animator: Canada - uncredited) 2004
Mulan 2: The Final War (Video) (additional assistant animator - uncredited) 2004
Mickey, Donald, Goofy: The Three Musketeers (Video) (additional assistant animator - uncredited) 2004

May I please have these credits removed so I don't appear to be claiming credit where credit is not due? Thanks!
