The reason they keep popping up is they are on 8 scattered individual episodes.
They get deleted at series level only to have the system reinstall them by default. I followed this phenomena on another topic. After the fifth or sixth time they figured it out and were all deleted simultaneously. That finally deleted it for good.

Thanks it looks like one of our tools is incorrectly re-adding the images to the episode pages creating the duplicates. I've removed them again now from all but the series title page and let the team responsible know.
