got a question about definition of Dialogue in a movie

heard a dialogue in a movie and i didn't know it's meaning and i wanted to ask it here....in the movie death wish iv the crack down, there is a scene when the mafian guy frank bagz argues with a woman in the car & she says 

"Fuck you and the horses you rode in on"... then man says you already did then woman replies "you came in second"

what does it mean "you already did .....you came in second"???

1.She has already fucked him.
2.Not sure on this, he came after her or he didn't get it up or his dick was smaller than a horse's...

I take it to mean something just slightly different, that the guy was saying the woman had already had sex with both him and the horse, and then she was saying having sex with the horse was more satisfying than sex with him.

Haha, also not sure what the second part means (almost like I copied it).
I don't really think they meant that she had sex with a horse. "The horses you rode in on" seems like an old saying to emphasize the previous ("fuck you", in this case).

Well, according to urban dictionary it means "get off your pedestal.
But, why did/how did he come in second? I guess it was just a remark that he was less endowed.
