How can I obtain license to use IMDb content for my commercial application/website?

I am interesting to sign up for IMDb Content Subscription Service. (http://www.imdb.com/licensing/subservicetc).

How do I Sign up and what are the fees.

Hello,

I am hoping you can you can direct me towards someone who can help my company start the process of acquiring a commercial use license. I went to http://www.imdb.com/helpdesk/contact_form, but I did not see a form that I can fill out. What am I missing?

When I click that link I see several paragraphs of text, followed by the form to fill in.

The top of the form is a blue bar with white text in it stating :
Compose New Message [current load: 746 open requests (what does this mean?)]
 There are only 4 fields in the form:
  • From:  (This is already filled in with your information and is not editable)
  • *Subject:   (required)
  •  URL: 
  • *Body text   (required)

If you've revisited that contact page but you still did not see the contact form as DavidAH_Ca described -- please make sure that you are logged-in to your IMDb account, and then reload that contact page and scroll down to find the form.
