My Great-Grandfather, Arturo Aguilar, was in a film called "Viva Villa!" starring Leo Carrillo, Wallace Beery and King Kong's Fay Wrey. He was the "Telegraph Operator #1" that said "I've got Mexico City." He has no credit for this role on IMDB. How do I go about getting his name added to the cast?

First (and important) question is that was your great-grandfather actually credited for his role (name appears in the credits)? Since it's so old film I assume not.
If he's not credited in the film, you probably should provide a screenshot of the actual scene where he appears and you should then add his credit with "(uncredited)" attribute.

I interpret your ironic post as meaning that IMDb's policy to add uncredited cast or crew members is not very practical.
For this film at least, it seems possible, since it is on YouTube, and I believe that, thanks to your question above, I've seen your parent at an important scene, starting at 1:02:23.

I believe in the 30s, only the lead actors were credited. Complicated things even more.

I'm old, but not that old, to identify the person in question. The scene is exactly that described by Tim Ballard, so I believe he will be able to determine the identity of his ancestor.
