Jim Caviezel was struck by lightning twice while making Passion of the Christ, including once when he was up on the crucifix. But I couldn't find anything saying the shot(s) made it into the movie.

Thanks Jen! These have all been added - yeah I never thought Caviezel getting struck by lightning but he did get hurt multiple times while shooting so I can double check to see if any of these made it in.

During the filming of the chariot race in Ben Hur, 1959, this shot http://www.imdb.com/title/tt0052618/m... wasn't a planned stunt. It was a real accident and the stunt man had to climb back into the chariot while it was still moving. Side note, the stunt man who survived that accident was the son of the famous stuntman Yakima Canutt who did the famous drag under the stagecoach in "Stagecoach" (1939)

- xXx (2002), the stunt with the parachute and Arab on the river, the stuntman died when his head hit the backside of the underside of the bridge. The footage was kept in the film as to honor the stuntman's last stunt. But the footage was later CGI-ed to make it look he safely passed under the bridge.

Collateral - Tom Cruise accidentally stepped on an office chair and fell. Michael Mann liked it so much he kept it in.
Never Say Never Again - When Fatima Blush (Barbara Carrera) goes to check on her patient Jack Petachi (Gavan O'Herlihy) she rips off his bandage and slaps him as hard as she can. Despite their rehearsing it was her "pulling" the slap to not actually hit, she actually made contact and knocked him over. She said her hand was sore afterwards.

The shot was left in and the impact Conrad's head
makes with the concrete floor of the sound stage

and still Gerard Butler gets no credit for everything!!! he was severely injured in multiple films: other than random injuries such as mentioned above from just doing his own stunts...pierce brosnan actually drove them into the wall in ''Buterfly on a Wheel'' where he sustained a permanent whiplash injury, he almost drowned and was hospitalized in ''Chasing Mavericks'', Dylan McDermott burned him with a cigarette that caught his makeup on fire in ''Olympus has Fallen'', he kept cutting himself and everyone else with the fang appliance in ''Dracula 2000'', i am sick of trying to force you idiots to appreciate anything but the mediocre drivel you slurp with spoons flowing out of corporate hollywood!
