It does not cost money to have credits listed on IMDb. That's what it is/does all on its own. So we can drop that part, for starters.
Could you post links to the pages that refer to you in some way?
I see:
Terry Ballard (I)
Do credits or any other data refer to you? If so, which?
You mention a "Terry Ball" is this credit referring you to you? Terry Ball (Art Department, Telling Lies in America (1997))
I don't find the other names you mentioned, " I AM TERRY JENE OR J BALLARD. YOU HAVE TERRY BALL AND OTHER'S"...can you clarify what you're saying?
Can you provide links (or describe the pages) for any other pages that have credits that refer to you?
What should your name appear as (as in the heading to a page listing your credits and other info)? By IMDb rules, it should be your name as it has appeared most often and recently in the credits of films/shows.

Just provide all the details and the contributors here will either advise you, or refer your corrections to the IMDb staff to have them fix the problem. But the more specific information you provide, the easier it will be to solve the problems.

The first one here is wrong. my birthdate is 6/13/35 and it states that I died?? nope still breathing Terry Ballard (I) (Art Department, Soylent Green (1973)) (2015) This one with the "goofus picture is not me )
Terry Ballard (II) if you need to use my middle name to make things easier my middle name I Jene. I never use it but you may if necessary..... Thanks for the help.

So Terry Ballard (II) is not you at http://www.imdb.com/name/nm7730769/, and he has no credits anyway. Can I assume that Terry Ballard (III) at http://www.imdb.com/name/nm7730834/, whose only credits are on a film titled White Wall (2015), is not you either?
That leaves you as Terry Ballard (I) at http://www.imdb.com/name/nm0050676/, except that you said that there is a wrong birth date and death date on that page. If the birth date and death date were removed on that page, would that resolve all your problems? Or are there other things that also need to be fixed?
You wrote above, "You have my credits on three different names. you have it under different names ...." Did you mean that there are three different pages called Terry Ballard? If that's the issue, then that's not a problem -- if there are three different people with the same name, then they are going to have three different pages in IMDb. But we need to make sure that each one of those three people gets their credits on their own respective page. And if you have any of your credits listed on IMDB on any other name page, then let us know what page it is, what name it is, and what credits those are.

The name Terry Ballard "Miller" came from my birth name "Miller"..... I was raised by my grandparents "Ballard".......from 1935 when my mother gave me up to her mother. I used the Ballard name all the way thru High School but when I joined the Army "Korea" they made me use my birth certificate name. When I got out I went back to Ballard which was never legally changed because in the 30s no one gave a damn....I only put that there in case it helped show who I am. Service was the only time I used Miller...........Hope that clarifies this whole mess. If IMDB checks back on their old records they will see that they had it correct then..... It was rude and uncalled for. I don't know what you mean by, "its (sic) still America" as this is not primarily an American site and exists on the internet, not in the US, but if you think that gives license to be a rude, jackass, I guess you have the right to be one...like I said...classy!
So, you're still suggesting that, other than the birth/death info, all the other stuff is accurate?

...and I assumed (wrongly, I guess) that this 'Terry Ballard' was Terry Ballard (I). I based this assumption on the fact that, according to his page on IMDb, he had passed away a few years before, he had worked as an actor and technical consultant for parts III & IV of the saga and, specially, due to the following important piece of trivia from his bio (if it's true, he'd be a key figure for the fans of the series).

And if this "In Memory of" credit goes to another person, do the Friday the 13th credits go with it?I'm not sure about this, because I think that the OP did work in the franchise. See this screencaps from Friday the 13th Part III (note the tatoos on his arms and compare them with the photos from above):

But, if he's "the Terry Ballard related to Friday the 13th", why does a documentary about Friday the 13th grant him an "in memory of..." credit?

To make matters even more confusing...

You are the one who came to this forum looking for help to correct a wrong information (glad you're alive, btw) and it's really sad that someone your age treats the people offering the help this way... Also, being a fan of the Friday the 13th franchise, it's double sad to receive that kind of response from you. I wouldn't like to think that's the reason why nobody in the documentary (@1h 40mins) pronounce your name when recalling who brought the famous hockey mask to the set... not to mention they had given you up for dead...
Believe it or not, our goal is not to annoy you, but to ensure that the database is accurate, and all the questions made along this conversation were to clarify some uncertain data.
Now that you've made clear that all the credits are correct, I hope an IMDb staffer sees this thread and deletes your date of death.

Please don't worry about my name being noted. Over 35 yrs and over 60k hrs in the business my name was one of the tops in my field. I don't expect any of you youngsters to know it. Re: your credits, it would really help if you could take a look at the page and specifically call out any credits that do not belong to you so we can move them to a different Terry Ballard (for example there is a production assistant credit for a film released in 2000 that looks out of place). If you give us all the details, we'll take care of all the appropriate corrections.
