Trying to Add New Titles

I've been trying to add some new titles over the past couple days but there's an error preventing them from being submitted.  When everything is green, I will hit submit but then it turns back to yellow where you add why you're adding the title.  This happens no matter how many times you fill it out and try to submit is. 

Is anyone else having this problem?
