why did my name get forgotten from the listing of cast and crew for a production

this is the second time that my name has been forgotten from a production, and both times I was a department head, and my assistants' names have not been forgotten.  How do you get the names of cast and crew, from a call sheet or from a crew list

Whatever title you are missing from please submit a credit to that title for yourself.

Also someone associated with the production submits the information.
Call sheets are not submitted so that an IMDb employee does it.
The call sheet info is used by the production staff on the title in question. They are the ones that missed your submission. Not IMDb.
