This can happen if you upload an image you've previously taken on your iPhone (rather than using the camera built into Instagram). To make sure there are no black strips across the top and bottom of your photo: after selecting the image from your Camera Roll or Photo Library (or other albums you may have), you will be asked to "Move and Scale" your image. At this point, enlarge the image to fill the square frame (using the zoom in or "reverse pinch" gesture) and tap the "Choose" button.

Of course, this means you will be cutting off the sides of your image but the design of Instagram has favored square images from day one so there's not much you can do about that.
