code invalid/already redeemed??

I am trying to redeem a code but it shows that it has already been used! I have NOT used it so how can this be fixed??
Myaccount shows this code was redeemed in June, but I have not been able to do that until now--so who redeemed it?? I need a replacement code since this one is not working and I did spend 1,000 points on it!
