Item never received, but didn't remember I'd ordered it. So, it was a long time ago (Jan. 2012)

I was reviewing the section "my account" and the sub-section "rewards" on your site while logged in with my account info. I was just looking back at the previous rewards I had spent point on and I came across a reward I had redeemed points for back in January, which I know is a long time ago, that I realized I never received. I obviously forgot that I had redeemed points for the item and so never noticed it didn't arrive. How can I get past shipping tracking information on an item to find out why I never received it. Was it not sent? Was it returned by the delivery service used to ship it? The item info: 1886 Distressed Coca-Cola Cap
