need fax number to send declaration of compliance

I just opened email and have a signed declaration to sign for prize. 23341. I cannot find a fax number to send the declaration to and I am at my 5 day limit I guess. Please help. I have never won such a prize before. and I want to collect my prize. I am trying to find a way to fax the declaration to you and there is no one to call at this hour.
