Are both config.xml and index.html in the root ("/") of your ZIP file or github input?

yes they are. Do you think that it would help to move all the icon images to the root file as some other people posted and change the file path in the the config file?

No that is a red herring. Check the path to the images. Make sure they are relative to the root.
Here is a working example/demo.

OK, here is some feedback:
1. Your www.zip contains two directories: /_MACOSX and /www
However, PGB expects no such directories. You should have both config.xml and index.html in the root of your git upload.
So, zip up whatever is currently in your /www directory.
2. Once your config is in the root, your paths to the icons will be correct.
3. The file format for the graphics is wrong, though. You have GIF images, renamed to .png
You can't do that. Create real .png graphics.
4. (unrelated) Your zip file contains assets and config entries for blackberry, bada, tizen, webOS, etc. That just bloats your input. Remove all that, because you will get apps for Android, iOS and Winphone only, anyway.

My name is Joe, I work with Vanessa. The zip folder is the www folder. I don't have any folders named either www or _MACOSX in the zip.
When building we are not using the git account but just uploading the zip. the git account is one folder level too high to connect to build directly.
I will recheck the images but they should all be actual .pngs already.

Hi Petra,
so we opened the www folder and then I zipped the contents directly inside of that instead of compressing the www folder prior to opening it and we are getting the same errors... any ideas?

Thank you for all your help. We were able to resolve the issue. We went back and reformatted all the icons from the psd files and re-saved them and then only zipped the files after the www file was open. Once we also changed the version number we were able to eliminate all of the errors.
